Using this site

By using this website, you agree to these terms. If you do not agree, please do not use the site. You may use it for lawful personal purposes, including reviewing clinic information, contacting the clinic and accessing approved booking or registration options.

Do not attempt to interfere with the site, gain unauthorized access, submit false or harmful information, or use its content in a way that violates another person's rights or applicable law.

Medical information and care

Website content is general information only and is not medical advice, a diagnosis or a substitute for an assessment by a qualified health professional. Using this site, sending an email or submitting a form does not create a physician-patient relationship.

Clinical decisions are made by the treating health professional based on an individual assessment. Service descriptions may not apply to every patient, and referrals, eligibility or additional information may be required.

Appointments and availability

All public appointments are requested through Ocean Online Booking, an independent third-party service. An appointment is confirmed only when Ocean or the clinic provides confirmation. The Halsey website does not hold or confirm appointment times.

Availability shown by Ocean can change until a booking is confirmed. Provider schedules, walk-in clinic capacity, services and wait times may also change. To change or cancel an appointment, follow the instructions in your Ocean confirmation or contact the clinic.

Appointment, late-arrival, cancellation and uninsured-service policies shown on this website may be updated.

Registration and booking forms

The prospective-patient form is for contact information, the required family-doctor preference and an optional Health Card number in its dedicated field. Halsey Medical Clinic is a walk-in clinic serving East York. Submitting the form does not place you on a waitlist, guarantee care with a family doctor, or guarantee a particular appointment time or clinical care. Information must be accurate and belong to you or a person you are legally authorized to represent.

A Health Card number may be entered only in the dedicated optional Halsey registration field. The Halsey registration form does not collect a UCI. When booking through Ocean, provide a Health Card, UCI or other sensitive identifier only if Ocean presents a dedicated field for it, and review its privacy notice before submitting. Do not include identifiers in notes, chat or ordinary email. See the Privacy & PHIPA notice for more information.

Accuracy, links and external services

Halsey aims to keep this website current and useful, but content may contain errors or become out of date. Contact the clinic to confirm information that affects your visit.

Links to booking, maps or other third-party services are provided for convenience. Those services are operated independently and have their own terms, availability and privacy practices. A link does not mean Halsey controls or guarantees the external service.

Website availability and content

The website is provided on an “as available” basis. Access may be interrupted for maintenance, security or technical reasons. To the extent permitted by law, Halsey is not responsible for loss arising solely from reliance on general website content or from the availability of an independent third-party service. Nothing in these terms limits rights or remedies that cannot lawfully be limited.

Intellectual property

Unless otherwise stated, the website's branding, design and original content belong to Halsey Medical Clinic or are used with permission. They may not be copied, republished or commercially used without prior written permission, except as allowed by law.

Changes, governing law and contact

These terms may be updated by posting a revised version and date. They are governed by the laws of Ontario and the applicable laws of Canada.
