Personalized care for conditions such as diabetes, hypertension and asthma. The team can monitor progress, review treatment needs and help you make informed day-to-day health decisions.
Typical appointment: 15 minutes
Your clinician will decide which assessments or treatments are appropriate based on your individual history and concerns.

Appointment policy
Plan ahead for your visit.
- Bring a valid OHIP card to every insured visit.
- Arriving more than 10 minutes late may require rescheduling.
- Cancel or reschedule at least 24 hours before your appointment.
- A $50 fee may apply to no-shows and same-day cancellations.
- Repeated no-shows may result in limited booking privileges.
- When a time is full, ask to join the waitlist for the next opening.
- Walk-in clinic patients are seen according to clinician availability.
