Service description

What to expect

Book an assessment for work-related medical concerns and required WSIB documentation. Please bring any forms, claim information and relevant records to your visit.

Your clinician will decide which assessments or treatments are appropriate based on your individual history and concerns.

Appointment policy

Plan ahead for your visit.

Bring a valid OHIP card to every insured visit.
Arriving more than 10 minutes late may require rescheduling.
Cancel or reschedule at least 24 hours before your appointment.
A $50 fee may apply to no-shows and same-day cancellations.
Repeated no-shows may result in limited booking privileges.
When a time is full, ask to join the waitlist for the next opening.
Walk-ins are welcome and seen according to clinician availability.
